I voted for the last - my wife does not go for pain in any way. When PMS sets in she can't even stand to have her breasts touched. Under normal circumstances gentle caresses are her preference.

Im not really sure what everyone else thinks is torture, but I have both nipples pierced twice (vertical and horizontal) I use clamps and bands on them and I love them to be twisted and pulled til I cry.
