Strap-on activities used to have kind of a taboo nature throughout the history, so it's hard to evaluate how common they actually were. But some records exist. There's a short article on Wikipedia about it, if you're interested

Probably not, because quality harnesses haven't been widely available until recently (I think the '70s-'80s was when they started becoming more commonly available).
