You could always use a dental dam.

Dental dams are the only super-safe option.

If you're fluid-bonded, the receiving partner can could clean up with an enema. When I was with a (fluid bonded) partner who enjoyed rimming, we were usually content to shower together and do extra-thorough cleanings while there.

Without a dental dam (or Saran Wrap / similar), it's never going to be sterile. You can choose which risks are worth pleasure, though.

Most doctor's and scientists have stated that it's impossible to clean the anus of all of it's bacteria, no matter how thoroughly you do it. With that in mind, some sort of a barrier will make it safer. One option I've seen suggested here is plastic wrap with lube on the receiver's end. It would give a similar feeling to the wetness of the tongue and keep you nice and protected.
