I, too, have TMJ. There's a lot of licking/touching that goes on. It also depends on what position I'm in, as the more I have to turn my head in some directions makes it harder on the muscles in my face that support the jaw. It's best if I kneel in front of him or if I kneel above him. If I lie down, it's more painful.
Another tip, if I plan on giving head, I will take some ibuprofen about an hour before to reduce any swelling I may have going on. It also makes it less sore when I wake up in the morning.
