I have with partners. It's pretty easy for me to then. But alone and with toys I haven't been able to achieve an orgasm. I've gotten close! But no dice. I think it's just a matter of finding the right toy with the perfect curve or pressure.

Yes my wife can have g-spot orgasms. Her first was with the Pure Wand - but we've had many since then. These days she's as likely to have an a-spot gusher - she likes the deeper penetration.
