For myself (5'2"), I wanted one that I could tell I had a bulge with it, so I went with the Small rather than the X-tra Small. I might even move up to the Medium if I were to wear it with jeans, but that's only if I wanted the bulge to be noticeable in any type of clothing. Overall, though, a Small should do fine.

Extra small feels so big to me, it's bigger than my girlfriend's (who is 7 inches, mtf) and I'd feel nervous getting the small.

Consider comfort. You could always start out smaller than you think, and work your way up- harder to go down I'd suppose.
