Yes, I was 12 and my grandfather opened the door - I don't know if he knew what I was doing (as I was under the covers and stopped as soon as the door started to open), but I had that zoned out blissful look on my face. I was so chilled out, I couldn't react too fast.

I've only been caught once. A college girlfriend who was staying with me and my family over Christmas break walked in on me without knocking (plus the lock on my bedroom door was broken). Fortunately I was under a down comforter and mostly clothed so she had absolutely no idea what she had walked in on. Now I have my lock fixed so that never happens.
That said I have caught more than my fair share of roommates.

I've had my sister and my best friend (separately) walk in on me while I was masturbating before.
