I have... twice. One was sex in a public place and I had sex in a park, although made sure no one was around. And I had a fantasy about golden showers and I sure have followed through on that one!

Wow!! Where to start? We have pretty much gone through all our current fantasies. We have- masturbated together, in front of and for each other, had sex in public without getting caught, made our home movies, I am a cam girl, I have sexy pics on certain sites for voting, we have an abundance of every sex toy we can think of wanting(and access to many more), had sex in the cars, beach, at a waterfall, quickies almost everywhere, and so many other hot n naughty fulfillments.....

I'd say more fantasies than not have been filled. I have been blessed with adventurous and open minded partners who have no problem with any suggestions.
In fact the rest of the fantasies I have (at least the big ones) will most likely be left unfilled because of logistics (or legality).
