I always rate. There's been maybe a handful where I either forgot to hit confirm or was unsure how to rate them.

I don't rate reviews if I don't get a lot of information out of it. It it was poorly done or I just didn't get anything out of it I just move on to another review of the same product.

Not every review, but most. I usually read a bunch of the reviews on the same product so I feel that'd be excess if I did every one I read.
