It doesn't look particularly painful, reminds me of a pirate ships anchor. The handle may be uncomfortable to sit down on, but I'd imagine the bubble shapes would be pleasurable rather than painful.
I didn't find it painful in the least. The length is filling and the little bumps simply add to the internal prostate massage. It is well worth the small price.
