An enema is an enema pretty much. Go with one having a pretty simple nozzle (trust me, you're really not going to be playing with it much, and a smaller smooth nozzle is easier to insert and remove). You also ideally want one that's easy to squeeze if you go the squeeze route. You might also want to check out a drugstore, and keep in mind that sometimes the enema bags are easier/ less trouble to use.

I have that ultra douche and it works just great. but as ScottA said, enema is an enema, there's not much of the difference. Maybe the way the bottom part feels in your hand, which would make it easier to sqeeze? I'd say so. Go with the ultra one.
