It is possible to use this with a double dildo; HOWEVER, you wouldn't be getting to use the bulb end of the double ended dildo.

I wonder if I'd be able to do any kind of sewing alteration for that to work without damaging integrity.

Hmm I am kind of surprised, as it seems the back panel should slide down enough to allow a feeldoe or similarly designed double dildo to be used between two partners...

It seems to me that the back panel will slide down enough to allow using something like a Feeldoe. This review seems to agree: Link.
