Have you ever gotten any pimples or breakouts on or around your genitals from using a water-based lube?

Once, but that day I had switched lubes halfway without washing (both water based). Not once since then, and now always wash b4 switching.

Once, but it hasn't happened again so far. I occasionally get pimples down there anyway, so I didn't think much of it.

I don't think I've ever had an actual pimple down there, just ingrown hairs, and not from lube. I mostly get them after having sex the same day as shaving, somehow what's left of the hair under my skin irritates me. Not so fun.
