The cock rings that I've purchased for fun really have been sized to go behind the balls.
The tension ring(s) that came with the pump prescribed by my doc for treatment of ED are so tight, they really could only go around the shaft. Even then, I cannot keep it on for very long, because it's so tight it hurts. The instructions say to definitely not keep it on longer than thirty minutes or physical injury may result.
