Here's a decent how-to:
link The exercisers, like the Desi, give you something to clench. If the balls are weighted, as most are, it also forces your muscles to clench more often in over to keep the balls inside, giving you stronger muscles faster. That's the theory, anyway. I've found it to be true enough.
