I'd rather have the toothbrush, but that's because I'm lazy about brushing my teeth and that would guilt me in to it. I wouldn't use it for anything but hygiene, ha.

I have a spinning toothbrush (and like it), but don't have a clitoris, so the clit vibe wouldn't be very useful to me.
