I have one with little nubs on the end. With that one, I'll sometime leave it off and just apply pressure with it.

The Lelo Mona has such a wonderful shape for G-spotting that I have sometimes used it without the vibration, mainly with my partner.

I always turn it on during use. But I only use clit vibes, which wouldn't make much sense to have turned off.
