The worst for me is getting a toy that just can't/won't get me off. I'm fairly easy to bring to orgasm with a toy, but give me something buzzy and it's just not happening.

I had a rabbit vibe that I loved. One day I was using it and all the of the sudden the inner components of the shaft that were all plastic shattered. Luckily none of the sharp plastic broke through the TPR shaft, but it was rather frightening to have my rabbit self destruct on me.

I had a buttplug almost get lost in me today because the base wasn't wide enough. Good thing it didn't go in too far and I was able to fish it out, or that would have been one embarrassing ride to the ER. Needless to say, that one's going in the trash and I'll be a lot more picky over the bases on my plugs.
