I agree with you about the issue of the non-flanged base. While the thickness might be good for those new to anal, it would be lackluster for more experienced play, and again the danger of not being able to get it out.
I think that perineal play would depend on your personal preference of stimulation. This one is overall pretty gentle, so if you need a stronger pulse, it wouldn't be the best. If a stronger vibration IS what you need, though, you might want to take a look at This massager. It's inexpensive, and was a little too intense for me. This one, too, is one speed, but has different caps you can use for different sensations. This one might be better for perineal play than the Pearls.
