This is a very adorable outfit with cute details. You can dress it up, dress it down, make it sexy, and make it appropriate. Wear it to a party, or wear it in the bedroom. It's an all-purpose costume and is generally made well. It was a bit big for me and the headband feature developed a crease because it was bent in the packaging, but overall it's one of the best costumes I've ever bought.
Pros:
Adorable, material stands up well to wear.

A little big for me, headband accessory developed a crease from a bend.
I bought this costume in a size SM a few years ago for Hallow's Eve, and I have worn it numerous times since. It's held up fairly well, and was made out of polyester, but was not rough or scratchy like some other costumes. The white fabric, ruffled skirt, and red detailing are just adorable.
I am 5'0" and about 95 lbs, and even the small size was a little large on me. It didn't show a lot of leg or cleavage, but it did look generally sexy overall. It was fitted, but not tightly, so you could see my waist but it did hang a little bit. I think someone who was even a tad bigger would have better luck with the sizing.
The hat is actually just a piece of plastic attached to a white headband. It didn't fit me quite right and when I got it, the plastic part with the symbol was actually a little bent in the corner. I folded it back the other way, and it evened out alright, but that was unfortunate.
I wore my costume with white thigh-high fishnets and white high heels. Lots of bright red lipstick. You could also zombify it up for Halloween with some intense gothic makeup, and a little more black accessorizing. I've done both.
As I said, it's not scratchy. It's pretty comfortable and I would recommend it for bedroom play. Maybe accessorize that with a stethoscope and tongue depressor, too. I'm not that into role playing, so I really just wear this for costume parties. It is a little translucent so I would wear white undergarments underneath for a mild effect, or black or red for a sexier effect! You could really intensify this with black fishnets and a garter belt, pigtails, lots of eyeliner, anything to match your style.
I don't wear the headband much anymore, because of the bending, but the outfit itself is just adorable and holds up pretty well for polyester. It hasn't developed any runs or pulls, and it hasn't bled in the washing machine or anything like that even though you supposed to hand wash it in tepid water with mild detergent. I got some cover-up on it at one point and put it on a low washing machine setting with mild detergent and it was fine. It is combustible but it self-extinguishes. I would probably stay away from flames with this costume.
