Un-groomed down there. Nothing is more gross. I can handle bad breath that's what mints and tooth brushes are for. But I will NOT be going down on someone if it means come up with a carpet in my mouth. *gag*

Of those listed, bad in bed would be the worst offender. Grooming/hygiene things can be stated, person feels a little embarrassed, and then it can be easily remedied. But bad in bed is only sometimes fixable

Bad in bed can be fixed with practice and communication.
Ungroomed pubic hair can be iffy, but it's not that hard to say "Time to trim the hedges!"
Butt pimples... Well, some people just have issues with breakouts and stuff.
Bad breath. That is something that bothers me, and I do not hesitate to tell him "You need to brush your teeth or go sit over there, away from me."
