I think it's planned for the most part, but it's usually just planning a few hours ahead (like, "during my shower tonight I want to have a little playtime.")

I masturbate because I'm agitated or need something to distract me from what's going on at the time. I don't think it's ever been just because I was horny. Most of the time, the toys only come out to play when the hubby is around.

Sometimes I catch myself planning it, but that is when I'm super stressed and need relief. Most often than not I'm spontaneous about it.
