I swear to God that before I ever ventured towards actual sex or masturbation, that I could think about something sexual to the point where I would get mild vaginal cramping that would eventually turn into an orgasm. I don't know if you mean like one of those intense leg shaking, out of breath, mind blowing orgasms or just a light, but really noticeable one? I've been able to easily just by thinking about it or if I remember the feeling of a recent O with my partner like the other night, I can have a very light one. I honestly don't think it's possible to have a earth shattering, leg shaking, screaming and moaning orgasm without even seeing it coming. I shouldn't say impossible because there is a condition, but it's extremely rare & it was actually shown on grey's anatomy.

I have no idea how, or why, but if I tense up my legs a certain way I can make it happen. But they are smaller orgasms. I have never had spontaneous orgasms, but I haven't heard of any treatments for it other than surgeries. Meditation might help. This issue is mainly a mental thing. Sort of like OCD.
