I usually don't worry about it. I try to help her reach her first one first, then she wants to make me cum, so whether its 5 minutes or 20 minutes isn't an issue. However, if I needed to, I would likely think about yard work or doing taxes or something equally awful.

When I was younger I had to do this more than I do now, but I would count backwards from 100. I figured if I made it all that way from the time that i felt like cumming than I did a pretty good job of trying to hold off. But now I rarely do this. If I'm feeling like I'm going to cum quick, then I will use her favorite dildo on my wife until she comes, and then I will make short work of finishing me off.

I don't delay orgasm by thinking about turn-offs (it seems like that would either sexualize my turnoffs, or put me out of the awesome sex headspace I was already in, and neither one sounds very good). Instead I sort of... grit my teeth and don't let myself cum til I know my partner's ready to?

Generally missionary and girl-on-top (and variants) it's easy for me to have multiple orgasms, but doggy style or anal I'll have an orgasm that's super-powerful, and I'll be too sensitive to continue. So usually we'll do something that gives me ~3 nice squirty orgasms, and then flip over, and I grit my teeth and hold off and let it build until we can cum together.

I have a male friend that does multiples as in 2 times 2 is 4. 4 times 4 is 16. 16 times 16 is...he knows all the way into the millions. lol.
I find that girls don't necessarily need to practice this as our orgasms don't mean "end of show" like with some men.

I've never seriously tried doing this because it just seems like it wouldn't work. Orgasms are so physical for me, I don't see how thinking of something different could prevent one (although thinking of something helpful can definitely get me over the edge if I'm having a hard time). If I want to delay an orgasm, I either ease up on the stimulation or relax my muscles--I seriously can't climax without my muscles pretty tense.
