If put on to tight it can cause pain or even damage to the penis/blood vessels. But that can be said for any improper fitting ring.

I have one that is similar (one less snap) and the only problem I have with it is when putting it on while hard, skin and/or hair can get caught in the snaps. Other wise it is a wonderful cock ring that can provide great restriction.
