Scariest Halloween costume suggestions

This year I want to go as the most f'd up charecter I can. Does anybody have any good ideas. Keep in mind I am a guy, but if I need to pop on a dress and some high heels I'm more then down for that. I just want something over the top and extremly creative. Any suggestions would help.
