I generally start with the most recent, but keep scanning titles until I find ones that I would want to comment on.

I look through recent threads (first couple pages) for ones that look interesting. I guess that's a combination of both. Sometimes I search for discussions, but usually only to read. I try to only respond to fairly recent posts - within a couple weeks to month, unless I think I can really add something.

I typically stick to the first page, but I only read things that interest me. Sometimes I answer, other times I find that my opinion has been stated enough that it isn't necessary. And still other times I'm not as interested as I thought. It really depends on my mood though.
