Massage Therapy: what's the general rule?

What is the general rule when you go get a massage? Do you take everything off or leave your underwear? If you take everything off do you get your butt massaged too? Does anybody use vibrators or stuff like that in real life massaging? Anybody knows? Thanks ahead for replying.
