I've seen a million warnings, on reviews and other places not to insert x toy anally due to not having a flared base or what have you. I haven't really done a lot of inserting. But why do you think the warnings exactly? Is it really that common? Do people really go to the ER that often with a coke bottle up there?
When it comes to inserting things anally, do people really just try any ol' thing up there?

I've done plenty of butt play and outside of fingers I would hesitate to put anything other than toys up my butt, and of those only ones that are either flared at the base or otherwise designed not to get lodged internally.
Things can definitely get sucked in and with a little lube involved relying on your grip alone seems like a pretty iffy plan. I would also hesitate to put anything up my butt that wasn't designed for that purpose. The people who put bottles, candles, vegetables, etc. up there are running a number of risks that just don't seem worth it when high quality toys in all shapes and sizes are easily obtainable.

Like Boink said, the warnings are real, not just to keep companies in business.
There are thousands of people going to ERs every year with things "accidentally" stuck in their bottoms, that need to be removed with tools only hospitals have.
The lower bowel has several reflexes, one of them to prevent you from... shitting your pants. That reflex causes a reverse of the muscles propelling stool forward, if, for instance, you can't get into a bathroom as soon as you feel the need to. (Although for your health, you should go when you need to.) This reflex will work on other things and actually pull the unsafe object into the rectum, where it may or may not be retrievable by home means.
Well made anal specific toys aren't that expensive. You can get a nice butt plug or a nice probe for under $20.00. I wouldn't take a chance on anything other than a real, made for anal play toy!

There is a reason that people and companies warn users against inserting certain things and toys analy. Things can get sucked in there and as said above and with lube it can be hard to remove.
The toys with a flared base are usually meant for anal use however some of the bases can be too small. To answer your question yes people do end up in the ER to have to have toys and other objects removed from them that were not meant to be inserted, I have a friend that works rounds in the ER and has already seen a few cases of such in the time that he has been working.

I'm going to say google one guy and a glass jar, but this is for adult eyes only. And not for the weak stomach. People put the craziest things in their ass. Be safe and only insert what is designed to go inside there.

A lot of it can be about knowing your own body.
If you've been playing around with anal toys for a while, and you know the maximum length you can insert in there, then you can get away with using any toy that's a couple of inches longer than this, even if it doesn't have a flared base, as there'll always be something protruding which you can grab onto and pull it back out with.

Until you really know your body well I strongly recommend sticking with things that will always have a bit sticking out. They're usually safe and you won't have a problem.
Once you really know your body you can experiment a little more, but you need to be careful - certain shapes are more likely to get stuck, and anything with a point or something that can catch on the intestinal folds or break is a NO-NO. You also need to be comfortable enough to go get help if you need it.

I know many nurses and I heard many stories about people going to the ER with things up the booty that got stuck, sucked in, etc. After hearing those horror stories I make sure there is always a flared base.

I cannot echo what everyone else is saying enough. I worked in the ER for almost a year. You would not believe the amount of people who had to be sent to surgery to remove things from there lower bowel.
Also another thing to consider is that many toys that say anal use are not good for anal use; such as ones with a very small neck. If the neck is not large enough the sphincter has no room to close around the toy. Also, I have seen many toys that say for anal use that do not have a large base on them. This is also dangerous.
