I chose the positioning pillowcase only because it comes with a silicone dildo as opposed to a vinyl one.

The pillowcase, I don't like heat sealed plastic, it tends to break at the seams. Think plastic kid pool.

I would say to try the 5 piece pillowcase 1st because its more stable and you don't have to balenace yourself along with the other reasons people gave you.
