So, I've had my first rabbit vibe for a few months and I've learned there are two ways you can position the ears of the clitoral attachment, above or below.
We have the Jopen VR12 - the 'ears' straddle my wife's clit for best effect. The bottom configuration will not work with that toy - and my wife doesn't think she would like it that way.

So far, I like the clitoral arm type that curves upward, so that I have a sort of thumb-like surface against my clit--for example, the Zini Zook. I like it to gently rest atop my clit.
I only have one rabbit style vibe, the Ina2, and the clitoral arm is just a rounded bullet so it usually lays right over/onto of my clit. I do have the Muse massager which has little bunny ear shaped bullets which i will typically use to sandwich my clit between with it resting slightly above the ears as shown in your first photo.
Ears don't work for me. Never in the right spot or strong enough. Bulbous like on my Jopen Vanity VR 7 is better or I'll move the ears and use my Hitachi Wand.
