Yes. My Fairy Mini burned the skin on my clit and made it bleed. Not fun. Lesson learned? Lube with wand massagers and avoid buzzy vibrations.

If I'm close to orgasm, there's a chance I'll lose some muscle control. This is not good when using a glass toy for thrusting. After bumping my cervix a few times (thankfully not hard enough to bruise), I swore off using completely firm toys when thrusting.
