The medium is an alright place to start, but the large is really good (haven't used it for pegging, but it's nice for other things too). The small is just TOO small; it's like the size of a finger...

I agree about the silk small being too small. I generally prefer smaller toys but even I found the silk small to be really underwhelming. I'd go with at least a medium.

the jump between the medium and the large is substantial, so I usually end up using the medium, I'd really like a medium and a half.

Depends on whether it's for vaginal or anal use. For anal, we went with the Medium. Small was too small and Large is way too big for us.
