What do I do if a product I requested to review never arrives?

I placed a request for a product for free in exchange for a review as part of the review program. This was August 31st. I keep checking the tracking number, and it still has not made it to the shipping provider over a week later. What do I do in this situation, especially if it is completely lost and never arrives?
Has anybody had this happen before?
