I don't know if it's still an option, but you used to be able to go into any post office and get one?

Ok, you got me curious so I went and checked, and yep they still do it. Here's a link:

They'll tell you to go online to fill out the personal information form and print it, schedule an appointment with the post office to get it processed (and your picture taken if you don't already have a photo) along with copies of driver's license and ss card and a birth certificate (and some other things), then you have to wait about six weeks or so to get the passport and your birth certificate back.

I got mine last year. It wasn't too painful, it's just bureaucratic tedium.

My post office will give you the forms at the window if you ask.

Processing times will vary. If you're planning to travel during the month of December, I suggest you start the process NOW. There is always a backlog during the holiday season.

The a large US Post Office usually has not only the forms but someone there to help with filling it out and answering questions.
