As my own response, I have experienced each situation listed above. I most often appear as myself, but am more of a third party. It's like I'm watching a TV show where I am the star.
Although, I do have quite a few dreams where I am there, but in someone else's body. I know it's supposed to be me, but I don't look like myself whatsoever.
