The doctor informs you that though you're able to easily communicate your decision now, you will be unable to do so in a matter of minutes. The doctor tells you that with absolute certainty he can save three of your senses, but two will be gone forever.

If I was asked when I was 10, sight wouldn't have been a option. My brother is blind, and for one day I had blindfolded myself with the help of the group home staff just so I could understand what he was going through. I couldn't believe how he managed to live the way he was and I found myself even more supportive of him.

I want to be able to hear, see, and feel. I would be really frustrated/sad to not be able to taste or smell anymore, but at least I could function in my day to day life.

Well, I'm blind as a bad as it is so losing sight wouldn't be the worst thing. I am very visually oriented, however. In all seriousness, I would have to say keep what's left of my vision, hearing and touch.
