Nearly - took the bullet out of a pacifier-style plug, not considering how much structural integrity I'd removed from the bottom of the toy (!). Retrieving that was as close as I care to worry about a lost item.

Nothing lost, though I have had things go all the way inside. A few deep breaths, relax and push and they come back out - that's why safely rounded toys are important. Flanges are even better, so you don't have the worry in the first place.

Yes, stolen. I had it out and laying on my bedside table and left the room. We had a bunch of people over that day (a couple I don't know) and I haven't been able to find it. I looked all over thinking maybe I've knocked it down somewhere with no luck. I've even changed the room around and still can't find it. I'm not nuts and I know what I did with it because it was my favorite toy. I hope to replace it soon.
I've also lost my Wanachi wand, but I lost that during moving. I had it in my old apartment and when we moved into the new one, I can't find it. Those are the only two I've lost, and I don't plan on losing anymore.
