I guess if the item being inserted was on the large side and it was just forced in - then yeah and possibly cause other damage.
If there's plenty of lube, use a smallish item at first to work up and take things slow, it should be fine.

I agree with Shellz31 - starting with a small toy, even though it may not look or feel filling, is the way to go. Even better, start with a lubed up finger and pay attention to how your body responds. I will still bleed a little some times and when that happens we just stop and cuddle. Start with something really small and work up. Lots and Lots of lube.
