I experienced very minor pain during orgasm for about 8 months, but it was never serious enough to go to the doctor. It is completely gone now. My semen volume seems slightly lower and the explosiveness of cumming seems less, but I'm 38 now, so it could be age too. I feel young though and stay in good shape. Most people can't believe I'm 38.
