There are many causes, probably too many to list here. The most common are probably medication side effects, underlying medical conditions (such as Diabetes), and psychological reasons. The only way he will know the cause and solution to the problem is by seeing a doctor.
Does he smoke or use recreational drugs? Both of these can impair blood flow to the penis.

From Wikipedia the main things that I would think might cause his issues would be either psychological, or related to something like smoking or anti-depressants (which are the most common other cause of erectile issues in younger guys).
Does he have any trouble with other forms of sex/masturbation, or is it only oral?
