Can't say I've ever heard of this happening. The glass used to make glass toys is incredibly strong. The only way I could see one breaking during use is if it had an enormous fracture that the person somehow didn't notice when they picked it up.
That said, even little scratches and fractures on glass toys are cause for concern, and you shouldn't use a toy if you see any damage like that. You could end up cutting yourself.

Yup, pretty hard to do during regular use. I dropped two from seven feet onto a steel table, one broke and one did not. The one that didn't break I dropped again, again no breakage.
Check them before use for scratches or small cracks.

I could see this happening if it was thrown across the room and landed on the concrete 3 floors below.
