I *think* I have taken one, but it has been so many years that I can't be sure. I have a grade school age kid, so that had made me consider taking one anyway. I think my taste for BDSM pushes it over the limit. I need to take one.
That said, I do have family in the medical profession, so I am not completely in the dark, but still---I would feel much better having been trained myself than trying to remember how to do things based on second-hand knowledge.

Yes: CPR, general first aid and wilderness first aid. I've led adventure trips and have also done a lot of solo hiking, cycling, xc skiing & backpacking.
I'm glad I've never had to deal with anything more than a little mild/moderate bleeding, mild altitude sickness and mild hypothermia. But it's good to know I could deal pretty well with pretty much whatever.

No, and I don't intend to unless it is a requirement for a job or something. I don't think any problems would come up in our play that could not be solved easily. If something bad happened to my partner, I would rather go to the hospital than try to solve the problem myself anyway.
