It depends. It seems like no matter what we're doing "one thing leads to another." Cuddling on the couch, and anytime we get in bed together guarantees we're going to have sex. We always try to have naps together, but never end up getting much sleep.
Sometimes we don't get to see each other for a week or 2 because we live in different cities and have conflicting work and school schedules. We usually send some dirty texts back and forth and kind of plan out a scenario for the next time we get to see each other. Then we usually jump each other as soon as we get in the door.

Sometimes I know all day that as soon as I walk in the door I'm going to attach him viciously...others it's a cuddle session that leads to a sexfest. Last night I was hanging on the couch getting a foot rub and before you know it we were naked....it just happens!!!!!

It's really all the above. The one I find that happens most frequently was "in the kitchen where you were SUPPOSED to be making dinner"... I'm not sure why but something about me making dinner in my panties gets him in a hustle for sex! We end up burning the food and having sex on the table. It's awful! Awfully good... that is...

It really depends on the day. Sometimes it's planned, sometimes we get in the mood when we're cuddling, sometimes it's the moment she walks through the door. It's always good, though.

Sometimes it's planned "I better get laid when you get home tonight!" often we play all day and it leads up to a low simmer until bedtime. Sometimes I wake up with him in my face or bouncing against my butt.

For us - all of the above depending on the day. Sometimes I plan on it all day, sometimes we're watching a ballgame together and the mood hits us, sometimes we're just having a wonderful chat and something he says puts me in the mood, and sometimes we do have date nights.

he has roommates, so we'll often go to his room 'to cuddle' or 'to nap' and that's a cue for 'go fuck.' we used to do it much more spontaneously. it doesn't happen spontaneously but once a month at most anymore.

I usually tell my dirty boy to get in the shower and hop in with him and it starts there and ends in the bedroom. Cuddling initiates it too though.
