My lady has never done it while I am wearing it and I haven't done it to her.

However, I must also point out that the two of us have only ever worn it once each. We are still pretty new to using it. And when I first used it on her, the dildo kept slipping out because the O-ring that came with the kit was slightly too large for the dildo so I had to find a replacement size.
