I haven't and wndered about this myself. If i wanted to buy someone something, or they me, would our names be exchanged, or is there "a buy for someone else" option?

I wish! Ha ha. No, no one's ever bought anything for me (yet?) and I've barely any money for myself so I've never bought anyone else something.

I've had gifts purchased for me and purchased gifts for others.

To purchase for others you want to look at their wish list to see what items are on it. Then you go to that item and click "buy as gift." It will give you a list of users that have that item on their public gift registry. Click on the user that you want to buy it for. It will add it to your cart. You can add anything else you'd like at that point. Then go through the checkout price as usual. At the top it just shows the user that you're buying for. The only information you learn about them is the state that they live in. It doesn't reveal their name or actual address.

You and the person receiving the gift get an email confirmation of the order as well as a shipping confirmation.

I have wondered about this as well. I haven't gotten anything but that's most likely because I haven't bought anyone anything either.

I found searching for items, then scrolling through the sometimes 50 or more pages of members, I just ask for their gift list public url and go from there. Where does Woman get all those wonderful toys? Is actually my latest blog post where I talk about gift registries and buying off them.

A few of my readers have wanted to get me things in this way and I've gotten things for others as well.

LOTS of fun and VERY sneaky sneaky!!

I did at Christmas, we had a secret santa swap and I participated. You only learn the other persons state but not their name or address or anything so you don't have to worry about any of that. We both get emails saying the order was placed so if you were trying to do it in secret, the secret would be out if they regularly check their email. I have my wishlist posted on my public profile because that was suggested when we did secret santa, but not everyone does that.
