Maybe you should try toys with bases? Toys slide out of me too, and it's normal. They're actually supposed to. Just keep thrusting them back in.

Depending on how you are using this toy, I've found it easier to keep in me if I'm sitting or reclining while masturbating. Or my wife would place her thumb at the base while giving me head. I could keep it in even when she was on top, just use a little creativity, you'll figure it out.

It depends on your toy and your anatomy. If you're talking about the Rocks Off series there are a couple of things that can help. The first would be to look at an anatomy drawing of the rectum/anus to see how things go together. Once you do that try playing around with the toy to see how/if you can get it inside without it pushing out too much. You can try putting it in backwards part of the way and turning it around or perhaps inserting while you're alternately "pushing out" and "sucking in".

Perhaps try a different type of toy? I'm not sure what kind you're using now, but there are so many shapes and sizes that most everyone should be able to find one that suits his/her individual body shape.

I normally find sitting helps. I find the nexus toys awesome in terms of staying in! I've got the glide, vibro and Excel and no issues with that. Tried the njoy pfun and that also tends to come out. Really down to anatomy and toys design.
