My wife has a IUD. And neither one of us has been with someone else, so we dont have to worry about STD's. I believe we are as safe as you can get, the only safer way would to go and get fixed.

I use condoms, only because I want to wait to have a child for a while. I also take birth control, but my medications lower their effectiveness. My partner and I also both have cold sores. I got mine from my ex boyfriend, and my fiance got his from his mother. She still terrorizes him about it today. So when either of us get one, we refrain from oral sex so we don't end up with actualy herpes on our genitals.

I only have sex with my wife, and she only has sex with me. It's not absolutely foolproof since you can get things from contact with other body fluids, but it does make STIs very unlikely.

We are poly but haven't had any lovers in a long time. We are very careful when we are with others. I get checked regularly from the Doctor. And he's had a vasectomy so we don't worry about birth control now.

To prevent STI's I always wear condoms with new partners even for oral sex. I make a date out of going to get an STI test with a new partner so we can confirm we are both clean before we stop using condoms and diaphragms for oral sex. For penetrative sex I always use both condoms and the Nuvaring. As effective as the ring is I'm pretty paranoid about pregnancy so I always have a back up method.
