These ankle are definitely as advertised, large. I generally have trouble with things fitting around my ankles, and I am able to wear these on the tightest hole. They have five different holes, which have about a 3/4 inch space between them. Because of the design of these cuffs, it is possible to punch more holes into them. This will not only allow them to work for someone with smaller ankles, but also will allow you to use them as wrist restraints as well. It will also allow for more precise sizing. I found that what I really needed was a hole in between two of the holes, which allows them to fit snugly without being too loose or too tight. I easily punched extra holes in mine with a belt hole punch, but you could also use a nail.

The bottom piece of the cuff is made from two pieces of leather which have been sewn together. The stitching is very well done. The inner part which touches the skin is very soft leather, and I have found it to be quite comfortable for extended wear. Attached to the thicker part of the cuff is a much thinner strip of leather. It is attached to the first layer of the bottom with metal rivets, four at the end by the buckle, and six by the D ring. Since the rivets are only attached to the first layer of leather, they do not touch the skin and will not cause any irritation. The buckle is very simple to use, just like a standard belt buckle. It is easy to fasten and unfasten, and the leather is quite pliable. The D ring is made of thick metal and is very sturdy. It can easily be used to attach rope, handcuffs, or to clip a spreader bar onto the cuffs.

During use, the cuffs are comfortable but also extremely sturdy. They will not accidentally come off, and I think it would be very difficult to break them without improper use.

The cuffs have a very faint leather smell to them, which you really won't notice unless intentionally smelling them.
