Some can be worn on just the shaft, some can be worn around the shaft and the balls. We use them to try and make sex last longer for my husband and to also hold a vibrator in the right spot for me to get off, but everybody's different

Alan's worn them both of the ways that Sam mentioned, and I sometimes put one on him backwards (with the vibrator under his sack) during oral to give him a little extra thrill. Cock rings aren't all the same though, so "what it does" for us really depends on which ring we use.
